REPLACEMENT OF CERTIFICATES
POLICY & INFORMATION
Continuing Education Certificates
1. All participants who successfully complete a continuing education course receive a course certificate of completion by postal mail.
2. Participants may request another certificate of course completion if needed as a replacement for a lost certificate. The cost for a replacement certificate is $ 5.00 per certificate.
3. Requests can be made in writing to: AIHCP, Inc., 2400 Niles-Cortland Rd. SE, Suite # 3, Warren, Ohio 44484. A personal check or money order is payable to: AIHCP.
4. Once a request is received along with payment, AIHCP, Inc. will prepare and postal mail the requested replacement certificate within 7 business days.
CERTIFICATION CERTIFICATES
1. All who become certified through AIHCP,Inc. and its subdivisions, receive a Certificate of Certification by postal mail.
2. Participants may request a additional certificate(s) or a replacement certificate if needed.The cost of a replacement Certification certificate is $15.00 per certificate.
3. Requests can be made in writing to: AIHCP, Inc., 2400 Niles-Cortland Rd. SE, Suite # 3, Warren, Ohio 44484. A personal check or money order is payable to: AIHCP.
4. Once a request is received along with payment, AIHCP, Inc. will prepare and postal mail the requested replacement certificate within 7 business days.
